News: EVOLVE: Weight Management Program Launched - Limited Openings

EVOLVE is an opportunity to get ready for that beach vacation, high school reunion or family wedding. Better yet how about improving blood pressure, cholesterol, diabetes, mobility, night sleep or reducing some of those aches and pains? In short, this program supports you on your journey to achieve the best possible version of yourself. By losing weight or maintaining a healthy weight you can enjoy a better quality of life.

How Does it Work?

 Individualized assessments and tracking across the full length of the Program.

Group Support – 12 weeks of weekly group sessions followed by 12 weeks of bi-weekly group sessions.

Individual and group support with a Provider Team made up of a Registered Nurse, Registered Dietitian/Nutritionist, Physical Therapist/Fitness Consultant.

Trusting personal relationships with your Provider Team Members so they can help you achieve your goals.

Email access to each Provider Team Member.

Medically focused, multi-disciplinary and hands- on approach seeking to build participant knowledge, motivation, and accountability to help obtain optimal health.

Group support will include celebrations for progress, troubleshooting challenges, weekly individual goal setting, educational topics pertaining to food, behavior modification, exercise, and more.

Access to a closed social media support community.

Strategies for post program sustainability.

The Provider Team Members will be in collaborative and immediate communication with Drs. Berliant, Lasin and Schaffel.

Who is Involved?

Providers:

Kristie Faller – Registered Nurse, Sheryl Gray – Registered Dietitian/Nutritionist, Bret Greenberg, Physical Therapist/Fitness Consultant.

Participants:

Exclusively for Forward Focus Concierge Medicine patients. Limited to ten participants who will be accepted on a first come first serve basis.     

When will it occur?

Monday, May 2 – EVOLVE Information Session – 5:30-6:30pm - Drs. Berliant, Lasin and Schaffel will review the program with you and introduce the multi-disciplinary Provider Team Members.

Friday, May 6 – Program Registration Deadline.

Thursday, June 9 - Program begins first 12 weeks. Weekly sessions - 5:30 – 6:45pm.

Thursday, September 1 – Program begins second 12 weeks. Bi-weekly sessions – 5:30-6:45pm

Thursday, November 14 – EVOLVE Graduation.

Where will EVOLVE take place?

All gatherings will be held in the Forward Focus Concierge Medicine suite unless otherwise notified.

EVOLVE Provider Team Members:

Kristie Faller – Kristie is a veteran registered nurse who has worked in the Lake Forest/Lake Bluff community for over 20 years. She joins the Team with a breadth of skills and experiences that are sure to enhance the personalized care the Program has to offer. She is particularly passionate about patient and family education and care management. She brings an energy and enthusiasm that will make our participants feel right at home.

Sheryl Gray – Sheryl holds a master’s degree in nutrition & counseling and a bachelor’s degree in nutrition and dietetics. She is registered nationally through the Academy of Nutrition and Dietetics and is licensed to practice in the State of Illinois. She has been asked to present at numerous conferences and seminars and has co-authored five nutritional publications. Sheryl has acquired a specialized approach to changing people’s eating habits. She designs for each patient a personalized approach to healthy eating. She does this through a balance of nutrition education, behavior modification and emotional support. Sheryl believes that making eating fun, flexible and more satisfying lead to a happier, healthy life…And the best diet is the one that’s personal and you can stick to!

Bret Greenberg – Bret currently holds his doctorate in physical therapy along with post professional certifications in orthopedics and manual therapy. But Bret’s passion lies in helping each person he works with make positive change in their life. Whether it’s better mobility, greater understanding of how to exercise or simply how to take care of your body, Bret is sure to use a creative, medically centered approach to help his patients reach their goals. He has extensive experience working with people who have never exercised in their life up to the professional athlete. He is patient, knowledgeable and fun! He can’t wait to be part of your journey to improved health.
